As required, Paycor is providing you with several important noticed related to your health plan benefits as follows:

Summary Annual Report (SAR) | Summary of financial information reported on our required financial filings.
SAR for Employee Benefits Plan SAR for 401(k) Retirement Plan |
Medicare Part D Creditable Coverage | The medical plans offered by Paycor are creditable, which means the prescription coverage is expected to cover, on average, as much as the standard Medicare Part D prescription drug plan. |
Employers Children's Health Insurance Program (CHIP) | If you or your children are eligible for Medicaid or CHIP and you're eligible for health coverage through Paycor, your state may have a premium assistance program that can help pay for coverage. |
HIPAA Notice of Special Enrollment Rights | Notice of your rights to enroll mid-year upon loss of other coverage, addition of a new dependent, or eligibility for CHIP premium assistance subsidy. The Paycor group health plan is subject to HIPAA's portability provisions and must notify individuals of their special enrollment rights, or the ability to make election changes outside of Open Enrollment due to a qualified life event. |
Women's Health and Cancer Rights Act (WHCRA) | Notice of benefit rights under WHCRA (providing for reconstruction after mastectomy) and any deductibles and coinsurance limits applicable to such benefits. Group health plans, like the Paycor medical plans, are required to notify participants of the plan's requirement to provide certain benefits relating to post-mastectomy surgery. |
Newborns' and Mothers' Health Protection Act (NMHPA) | Notice of benefit rights under NMHPA (providing for minimum length of hospital stay in connection with childbirth). |
HIPAA Notice of Privacy Practices | Notice that describes your rights, the Plan's legal duties with respect to protected health information (PHI), and the Plan's uses and disclosures of PHI. |
Exchange | Notice providing information about health coverage available through Exchanges (Marketplaces). |
Outbreak Period | The U.S. Department of Labor and IRS announced temporary extensions of certain plan deadlines during the COVID-19 pandemic. |
Summary of Benefits of Coverage (SBC) | Summary document describing key features of Paycor medical and pharmacy plans. |
Summary Plan Description (SPD) | Summary plan document describing benefits, rights, and obligations under the Plan. |
Health Savings Account (HSA) Automatic Pretax Contribution Notice | Paycor has implemented an automatic election for Associate pretax HSA contributions meaning your pretax HSA election from 2023 will carry over to 2024 unless you submit a request to change your HSA election. You may request to change your HSA election at any time throughout the year. |
No Surprises Act | Notice of your rights and protection against surprise billing or balance billing. |
